Dear Mr. Keppler:
This letter serves as a final report pursuant to 10CFR50.55(e), concerning a breakdown in the Quality Assurance Program of the Carboline Company.
Initial notification that this problem was being evaluated by The Cleveland Electric Illuminating Company (CEI) was made to Mr. P. Pelke of your of fice on April 8, 1983, by Mr. E. Riley of CEI.
This report includes a description of the deficiency, an analysis of the safety implication, and provides the results of CEI's evaluation.
Description of Deficiency Based on the findings of Metalweld Audit #830321 (March 21, 1983 - Vendor Audit of Carboline Company, St. Louis, MO), it has been determined that Carboline's QA Program, in terms of implementation, documentation, and asaur-ance of consistent product quality, is not adequate at present to provide a Quality Assurance Program conducted in accordance with the requirements of 10CFR50, Appendix B.
Analysis of Safety Implication The primary function of the coatings manufactured by the Carboline Company is to facilitate the control of contamination, as well as to protect surfaces from corrosion. The critical relationship of coating work in the Primary Containment Structure to the Engineered Safety Systems (ESS) is that it is essential that the coating remain in place and intact after a Loss of Coolant Accident (LOCA) in order not to compromise the ESS.

Corrective Action Metalveld, accompanied by a CEI representative, has re-audited Carboline for verification of their corrective actions. The audit was completed April 28, 1983. The result of the audit is that Carboline has initiated all the required corrective actions (i.e., formula reviews, management review, manual additions, new procedures, indoctrination and training). Carboline's review of the formula changes indicated no problem with the materials supplied to the Perry Proj ect. Another audit will be scheduled prior to September 30, 1983, to assure Carboline's implementation.
