FINAL REPORT - DER 83-12 DEFICIENCY EVALUATION 50.55(e)
ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNITS #1 AND #3 1.
Description of Deficiency General Electric (GE) has advised Bechtel Power Corporation that an inherent operational Deficiency may exist in normally open electric interlocks (GE Part No. CR206D022ZACN) used with Class 1E, Size 2, Starters (see attached GE Letter, dated February 9, 1983). These Starters are located in Class 1E 480 Volt Motor Control Centers (MCC's) supplied by GE.
The identified MCC's (1-E-PHA-M31, 1-E-PHB-M32 and 3-E-PHB-M34) are used in conjunction with the following safety-related equipment:
1.
Diesel Generator Control Room Essential AHU Fans M-HDA-A01 and M-HDB-A01 2.
Essential Chilled Water Circulation Pumps M-ECA-P01 and M-ECB-P01 3.
Fuel and Auxiliary Building Essential Exhaust AFU Heater M-HFB-E01 This condition was conveyed to Bechtel after the GE equipment had been received and all of the qualification documentation completed.
II.
Analysis of Safety Implications This condition is evaluated as Reportable. The subject Deficiency could potentially result in the malfunction of the safety-related equipment itemized above, coastituting a safety significant condi-tion as defined by 10CFR50.55(e).
III.
Corrective Action Design Change Package No. ISE-PH-034 and No. 3CE-PH-034 will be issued to implement GE's corrective action plan.
In addition to reportability under 10CFR50.55(e), Bechtel PVNGS Project considers the Deficiency to be Reportable under the require-ments of 10CFR Part 21.
Deficiency Evaluation Report No. 83-12 addresses the reporting requirements specified under 10CFR21.21(b)(3) with the exception of Sub-Part (vi) which requires the number and location (customers and/or facilities) of other possible defective equipment. A copy of this report will be sent to GE requesting their review for reporting under 10CFR Part 21, including number and location of all components supplied.

Replacement interlocks are presently being certified at our Mebane Manufacturing Facility. However, before we send these replacements to the site, we will need the necessary Bechtel identification (i.e.,
DCP #, NCR #, etc.), so that they can be received at the Bechtel Warehouse.
t Please provide me with this information so that we may proceed with shipment.
In addition, General Electric Engineering Services will provide technical direction to direct Bechtel craft labor in the interlock changeout and therefore, I would appreciate knowing who to contact to schedule the work.
