Dear Sir:

The hTC was notified of a Potentially Reportable Deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30)

days.

Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this information will be finalized by August 1, 1983, at which time a complete report will be submitted.

POTENTIAL REPORTABLE DEFICIENCY l

Potential Problem While performing a pre-service examination, it was discovered that seven (7) Mechanical Shock Arrestors were not properly performing their function as follows:

A.

NCR's No. S-216-1M and No. S-225-1M document Pacific Scientific Models PSA-1/4 (350 lbs.) and PSA-3 (6,000 lbs.) Snubbers frozen or locked up in position.

B.

NCR's No. S-227-1M, No. S-239-1M and No. S-252-1M document Pacific Scientific Models PSA-1 (1,500 lbs.)

and PSA-1/4 (350 lbs.) Snubbers binding up during part of the stroking cycle.

C.

NCR's No. S-253-1M and No. S-254-1M document Pacific Scientific Models PSA-3 (6,000 lbs.) and PSA-10 (15,000 lbs.) Snubbers rubbing against insulation clips.

II.

Approach To and Status Of Proposed Resolution The NCR's for Items A and B are being dispositioned replace the installed Snubbers with Snubbers of equal design and return the deficient Snubbers to the vendor.for repair and evaluation (Interim Disposition). The NCR's for Item C are being dispositioned " rework",

to remove the interference (Interim Disposition). Bechtel Engineering is corresponding with Pacific Scientific to identify the cause of this condition.

III.

Projected Completion of Corrective Action and Submittal of the Final Report

. Evaluation of this condition and submittal of the Final Report is forecast to be completed by August 1, 1983.
