Dear Sir:
The NRC was notified of a Potentially Reportable Deficiency in Reference (A), an Interim Report was transmitted by Reference (B) and a Time Exten-sion was requested by Reference (C). At that time, it was estim.ted that a Final Report would be available by May 15, 1983.
i Due to the extensive investigation and evaluation required, a revised Interim Report is attached. It is now expected that this information will be finalized by August 23, 1983, at which time a complete report will be submitted.

INTERIM REPORT, REVISION 1 - DER 82-76 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNITS 1, 2 AND 3 I.
Potential Problem Four (4) Target Rock Valves procured by Combustion Engineering (CE) and delivered to PVNGS were returned to CE for additional qualifi-cation testing in accordance with NUREG-0588. For this program, two (2) one-inch (1") Target Rock Model 77L-001 and two (2) two-inch (2") Target Rock Model 77L-003 Solenoid Valves were received. These Valves were intended for use as the Safety Injection Tank Vent Valves (Model 77L-001) and as the Pressurizer Auxiliary Spray Valves (Model 77L-003).
Inspections conducted prior to the test programs as well as the results of the test program itself revealed a large number of anomalies including:

Insulating Washer Incorrectly Installed b.
0-Ring Seals and Rubber Grommets Missing c.
Electrical Short in the Solenoid Leads d.
Lead Wires Damaged e.
Valve Unable to Seat Properly Due to Soft Seat' Ring Coming Out of Retention Groove f.
Limit Switch Valve Position Indicator Failures CE has filed a 10CFR Part 21 Report concerning the above defects.
II.
Approach To and Status Of Proposed Resolution The following resolution plan is summarized from CE Letters V-CE-18152, dated March 22, 1983 and V-CE-18162, dated March 23, 1983:

CE is continuing their efforts with the environmental qualification of the subject Valves. Test Plan CE No.
TP-ESE-Oll, Revision 2, (the Target Rock Environmental Test Plan) has been superseded by Test Plan CE No. TP-ESE-024, Revision 1.
Bechtel has reviewed the revised

test plan document in accordance with Internal Procedure No. 5.22 and returned it to CE with comments.
2.
As an alternative, CE is performing an investigation into the procurement of other Valves as replacements for the Target Rock Valves.
