. Title 10 Code of Federal Regulations Part 21 Gentlemen: -

MCC Powers has determined that a construction defect exists regarding shielded multiconductor cable installation in NSR HVAC Control-Panels supplied by MCC Powers. See attached list.

During the installation of this cable, when the exterior covering was removed, the insulation of various separate c:ntactors has been damaged due to cutting too deeply into the covering material. .The damaged insulation may possibly cause a loss of or reduced output of signal from the asso-ciated instruments if the. wiring were subjected continuously to a high moisture atmosphere, therefore should be repaired or reworked.

The information that the defect does exist at the LaSalle Station was obtained on April 15, 1983 and verbally trans-mitted to Mr. Harvey Westcott of the NRC Region Three office the same day.

MCC Powers has had all of the suspect panels inspected and is in the process of issuing NCR's for the applicable sites.

MCC Powers is currently engaged in construction at the Limerick and Clinton sites and has informed all applicable site construc-tion Quality Assurance Managers of the problem.

MCC Powers is recommending that if suf ficient excess cable is available in the panel that the defect be removed and the cable be reterminated. If there is insufficient cable available then a Ray Chem type WCSF-N shrink-on insulating material is to be installed over the defect. This material is qualified by the Manufacturer to IEEE-323-1974 and IEEE-384-1974.

As a third alternative, a replacement of the affected cable may be accomplished.
