,e b,v DUKE POWER COMPANY McGUIRE NUCLEAR STATION INVESTIGATION OF UNIT TRIPS OBJECTIVE This directive specifies the requirements for investigation of unit trips.
IMPLEMENTATION Upon a Unit Trip, the Superintendent of Operations or his Operations Duty Engineer will initiate an investigation of sufficient depth to determine the cause and the effects of the trip.
Prior to restart of the unit, the Superin-tendent of Operations or his Operations Duty Engineer will assure that:
1.
The cause of the trip is known or has been investigated to the full extent possible while remaining in the shutdown condition.
2.
The effects of the unit trip are such that a resrcrt may be made without further review.

Any equipment or component malfunctions observed, important to safe I
operation, are corrected prior to restart.
APPLICABILITY The effects of the Unit Trip will be evaluated and documented by an Engineer from the Performance Section assigned to the unit.
Where appropriate, the Superintendent of Operations or his Operations Duty Engineer will request that the Engineer from the Performance Section take full responsibility for the trip investigation and upon completion make recommendation to the Superintendent of Operations or his Operations Duty Engineer concerning restart of the unit.

part of the decision to restart the unit unless the Superintendent of Operations i

or his Operations Duty Engineer specifically requires this information on any given trip.
The decision to restart rests with the Operations Group.
DESCRIPTION In any investigation of a unit trip, the ~ Performance Group investigator will have available to assist him, the appropriate individuals from the station organization to pursue the various facets of the trip.
The investigator may call a meeting of appropriate personnel to define the investigative program and make assignments with appropriate time limits for completing the investigation and taking of corrective action.
Full cooperation of all Station personnel must be provided to ascertain what happened so that the appropriate action for returning the unit to service is achieved.
The Technical Services Engineer should be notified for any unit trip classified as a reportable occurrence.
The Station Review Committee will make recommendations on any incident reports resulting from unit trips, r'~'y PROCEDURE t

l The investigator, after completing the investigation will:
1.
Advise the Superintendent of Operations or his Operations Duty Engineer of his findings and recommendations for followup actions that may or may not require resolution before unit startup.
2.
Prepare a written report detailing the investigation. (This may be done after recovery from the trip).
