PVNGS UNITS 1 & 2 I.

DESCRIPTION OF DEFICIENCY-11e Waldinger Corporation is the prime HVAC Subcontractor to

'Bechtel Power Corporation and in turn purchases instruments from Honeywell. Honeywell employees have notified The Waldinger Corporation of the following irregularities in installation of instrumentation ~in the Units-l 'and 2 Diesel Generator Buildings:

'four (4) in Unit 1 and five (5) in Unit 2.

. Instruments have been found deliberately damaged (bent probes) in an attempt to cover up errors.

These irregularities indicate that the other instrumentation 4

provided'and installed by Honeywell may not have had all the engineering acceptance criteria identified.

II.

ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as Reportable.

Improper application and/or damaged instrumentation devices could possibly cause the safety related HVAC system to malfunction or to be improperly operated. This condition constitutes a safety significant condition as defined by 10CFR50.55(e). The Waldinger Corporation has previously notifiad the NRC of this condition via a 10CFR -Part 21 Report on 12/3/82.

III.- CORRECTIVE ACTION 1.

All Honeywell instruments (Q, R and S-Class) in Units 1, 2 and 3'will be' inspected by Honeywell in accordance with Honeywell Project Engineering Procedures 2.10, 2.11 and 2.12.

In addition, Waldinger Quality' Control will perform a reinspection of all Q-Class and a sample of R-Class Honeywell instruments (all units).

The condition is' evaluated as an isolated event.

Identified individuals have been dismissed-from the jobsite.

All Honeywell employees have been instructed in the

requirements to report discrepant conditions in accordance with Honeywell Procedure QSP 15.1 rather than attempt to cover up an error by damaging instruments.

Copies of this report will be transmitted to The Waldinger Corporation who,' in turn, will notify the responsible Honeywell individuals.
