Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time,-it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Rep' ort is attached. It is now expected that this information will be finalized by May 3, 1983, at which time a complete report will be submitted.

1. POTENTIAL PROBLEll General Electric (GE) has advised Bechtel Power Corporation that an inherent operational deficiency may exist in normally open electric interlocks (GE Part No. CR206D022ZACN) used with Class 1E, Size 2 Ftarters (see attached GE letter d?ted 2/9/83). These starters are located in Class 1E 400 Volt !!otor Control Centers (MCC's) supplied by GE. The identified 11CC's (1-E-PIIA-!Dl,1-E-PilB-M32 and 3-E-PilB-t-04) are used in conjunction with the following safety related equipment:
1) Diesel Generator Control Room Essential AIIU Fans M-llDA-A01;
2) Essential Chilled Water Circulation Pumps M-ECA-P01 and

!!-EC B-P01;

3) Fuel and Auxiliary Building Essential Exhaust AFU Heater M-IIFB-E01.

II. APPROACil TO AND STATUS OF PROPOSED RESOLUTION This condition was conveyed to Bechtel after the GE equipment had been received and all of the qualification documentation completed.

Design Change Package 1SE-Pii-034 and 3CE-PH-034 will be issued to implement GE's corrective action plan.

III. PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBI!ITTAL OF Tile FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by May 3, 1983.

msw Please find attached a list of 480V MCC equipment located at the Pahy"j Verde site having Size 2 Starters containing normally open add-on ~

interlocks that potentially may malfunction.

Replacement interlocks are presently being certified at our Mebane Manufacturing Facility. Howevcr, before we send these replacements to the site, we will need the necessary Bechtel identification (i.e.,

DCP #, NCR #, etc.), so that they can be received at the Bechtel Warehouse.

Please provide me with this information so that we may proceed with shipment.

In addition, General Electric Engineering Services will provide technical direction to direct Bechtel craf t labor in the interlock changeout and therefore, I would appreciate knowing who to contact to schedule the work.

Your assistance in providing me with the necessary information is greatly appreciated.
