l PIP 2-C94-1032 Vibration and Loose Parts Monitoring System Channel 9 On July 19, 1994, Channel 9 (S/G A channel 2) was declared inoperable. This channel failed the acceptance criteria for background noise during performance of PT/0/B/4600/03 (Loose Parts Spectrum Comparison) . A work request

(#94028984) was generated to repair this channel during a future outage of sufficient length since containment entry is required for this work.

Coverage for S/G A is still provided by Channel 5.

Since these channels have been inoperable for more than 30 days, they are reportable to the NRC pursuant to Technical Specification 3.3.3.9.
