SPECIAL REPORT CATAWBA NUCLEAR STATION, UNIT 2 PIP 2-C94-1032 1
Vibration and Loose Parts Monitoring System Channels 4 and 6 On July 5,1994, Channel 4 (Upper Reactor channel B) and Channel 6 (S/G B Channel 1) were declared inoperable. The LEDs associated with these channels did not light during the performance of PT/0/B/4600/03 (Loose Parts Monitor Spectrum Comparison).
Work orderd 94051250-01 and 94051251-01 were generated oy Operations to address this problem.
Since these channels have been inoperable for more than 30 days, they are reportable to the NRC pursuant to Technical Specification 3.3.3.9.
