The nondestructive examinations utilized VT, PT, MT, RT, and UT techniques. All the examinations were performed in accordance with written procedures that conform to the applicable sections of the ASME Boiler and Pressure Vessel Code. The folicwing is a summary of the recordable indications found during the examinations and evaluated by PBNP personnel. 94Ul-7P001, Class 2 Spring Hanger, lH-01A Datasheet 94U1-750P014 Inspection revealed that the load flange was 3" off center to the base plate and the cold load setting was higher than the required range. The off center load flange meets design requirements. The IEB 79-14 walkdowns have identified this support for removal. The operability of the system is not affected. No corrective actions are required. 94UI-7P002, Number not used. 94Ul-7P003, Number not used. 94UI-7P004, Class 2 piping support, SI-87 Datasheet 94Ul-750P018 Inspection revealed lack of complete thread engagement on a nut that attaches the support beam plate to the wall. The nut is tight and the condition has been prevalent since construction. This condition has been previously analyzed in CR 91-094 and was found to be acceptable. No corrective actions required. 94U1-7P005, Class 2 piping weld, SIS-04-SI-1005-04 Datasheet 94U1-451P002 A 0.062" crater crack indication on weld SIS-04-SI-1005-04 detected by PT. This pin provides support in the shear direction only and design drawings call for a nut / bolt combination or a pin with cotter pins to prevent lateral movement. This condition meets the design requirements and was evaluated as acceptable with no corrective actions required. 94U1-7P008, Class 2 Spring Hanger,3H-02 Datasheet 94U1-750P055 Inspection revealed that the required cold load setting could not be obtained with the installed spring can range and the cold load setting was higher than the required range. It was found that a size 7 spring can was installed whereas the design drawings called for a size 5. A Condition Report (CR 94-167) was initiated to resolve the conflict. CR 94-167 determined that the operability of the spring hanger and the system was not affected. The drawings will be revised to reflect the "as built" condition. 94Ul-7P009, Class 2 Spring Hanger,4H-04 Datasheet 94U1-750P029 Inspection revealed the load flange was 2" off-center from the support plate. This meets the design requirements of the spring hanger and no actions are required. 94U1-7P010, Class 2 Spring Hanger,5H-01 Datasheet 94U1-750P030 Inspection revealed the load flange on the spring can was missing and the support was 1" off-center of the base plate. This meets the design requirements of the spring hanger and no actions are required. 94UI-7P0ll, Class 2 Pipe Support,6H-02 Datasheet 94U1-750P032 The slide plate was reported 2" off-center from the floor slide plate. This indication does not violate the acceptance criteria of ASME Section XI, IWF-3410 and was found acceptable during analysis performed during IEB 79-14 walkdowns (Stress Cal. No. PD-GGS-001 Rev. 0). No corrective action required.

The remaining nuts on the snubber were tight and the operability of the snubber was not compromised. Investigation revealed that the most likely cause of the loose nut was from insufficient tightening during snubber surveillance testing conducted during 1993 (U1R20). MWR 941097 was issued to tighten the nut, j 94Ul-7P013, Class 2 Spring Hanger,6H-33 Datasheet 94U1-750P040 Inspection revealed that the load on the spring can (1300#) was lower than the required cold load setting. The improper load violated the acceptance criteria of IWF-3410 (a) (5) but has existed since construction as evidenced by the previous inspection reports. Since the load is within the operating range of the spring can, the operability of the spring hanger and the pipe system is not affected. The spring can will be readjusted under MWR 9402427. 94U1-7P014, Class 2 Piping Weld, AC-10-RHR-1001-08 Datasheet 94U1-451P005 Linear indication at toe of weld AC-10-RHR-1001-08 detected by PT appears to be a hot crack in the weld area. The indication is 0.12" long and the ASME Section XI acceptance criteria is greater than 0.2". The acceptance criteria has been met, therefore no corrective action required. 94Ul-7P015, Class 3 Pipe Support, H-313 Datasheet 94U1-750P045 Inspection revealed the upper and lower locknuts on the tension bar to be loose. This condition was previously identified and was not repaired. This support is exempt from inspection requirements in the ISI program. RRM 94-0037 was initiated to tighten the locknuts. 94Ul-7P016, Class 3 Pipe Support, H-95 Datasheet 94U1-750P050 Light general corrosion was reported on the riser clamp of the floor support. The i corrosion is a result of humidity and general moisture in the air. The corrosion does not compromise the structural integrity of the support or the pipe and is acceptable. There is no corrosive environment present to cause unusually high corrosion rates. No corrective actions required. i }

Condition Report 94-101 was initiated to investigate the root cause and corrective actions. CR 94-101 determined that the operability of the system is not affected. Additional examinations in accordance with IWF-3410 were performed with no additional discrepancies. This support will be subject to successive examinations. l 94Ul-7P020, Class 2 Pipe Support,3H-03 Datasheet 94U1-750P057 Inspection revealed that the base plate was suspended 3/4" off of the grout pad. This support was previously analyzed by calculation No. 85 of the IEB 79-14 walkdown. The condition identified was acceptable with no actions required. 94U1-7P021, Class 2 Spring Hanger,2H-02 Datasheet 94U1-750P033 Inspection revealed that one of the two spring cans was slightly below the required load range. The improper load range violates the acceptance criteria of IWF-3410 (a) (5) but has existed since construction as evidenced by previous inspection reports. The spring can will be readjusted under an MWR 9402424. The operability of the spring hanger and the system is not affected.

The spring can will be readjusted under an MWR 9402425. The operability of the spring can and the system is not affected. 94UI-7P024, Class 2 Spring Hanger,3H-06 Datasheet 94U1-750P064 Inspection revealed that the actual load was lower than the required load range. The improper load range violates the acceptance criteria of IWF-3410 (a) (5) but has existed since construction as evidenced by previous inspection reports. The spring can will be readjusted under an MWR 9412428. The operability of the spring can and the system is not affected. 94U1-7P025, Class 3 Pipe Support, S-524 Datasheet 94U1-750P081 Inspection revealed general corrosion on the pipe support and pipe saddle. The corrosion is a result of humidity and general moisture in the air. The corrosion does not compromise the structural integrity of the support or the pipe and is acceptable. There is no corrosive environment present to cause unusually high corrosion rates. No corrective actions required. 94Ul-7P026, Class 2 Pipe Support, H-203 Datasheet 94U1-750P068 General corrosion was reported on the support with a higher amount on the base plate. The indication was as a result of a surface irregularity in the welded area and was not a flaw like indication. The surface irregularity was mechanically removed (RRM 94-0030). 94Ul-7P029, Class 2 Pipe Support, SI-80 Datasheet 94U1-750P090 Inspection revealed a loose nut on the U-bolt support. The nut was tightened and the support reinspected. An additional examination, in accordance with IWF-2430, was performed. This support will be subject to successive examination in accordance with IWF-2420. 94Ul-7P030 - Number not used. 94Ul-7P031 - Number not used. 94Ul-7P032, Class 1 Pipe Support,42-20-PSSA Datasheet 94Ul-750P112 Inspection revealed that the pipe support was not attached to the building structure in any manner. Further investigation revealed that the support is actually a guide / restraint and does not support any load. This item will be removed from the ISI database. 94U1-7P033 - Number not used. l

Further evaluation revealed that the concrete was not part of the support. The support meets the acceptance criteria of IWF-3410. The cracked concrete has been previously evaluated by WE civil / structural engineering to not have any operability concerns or affect on the loading of RC-04. No corrective actions required. 94Ul-7P035, Class 1 Hydraulic Snubber, HS-13 Datasheet 94U1-750P114 Inspection revealed three nuts on the support plate did not have complete thread engagement. The nuts did not show any evidence of loosening and remain tight. Evaluation of this condition indicates that there is adequate thread engagement to provide the appropriate support. No corrective actions are required. 94Ul-7P036, Class 1 Piping Weld, RC-10-AC-1001-06 Datasheet 94U1-451P038 Dye penetrant inspection revealed two rounded indications that appear to be porosity in the welded area. These indications meet the ASME Section XI acceptance criteria. No corrective actions required. 94Ul-7P037, Class 1 Spring Hanger, SI-09 Datasheet 94U1-750P126 Inspection revealed that the support was offset and that two non-load bearing nuts were loose. The offset support is a condition that existed since construction. The nuts were loose to accommodate the offset. MWR 9401939 was initiated to correct this condition. The operability of the support and the system is not affected. 94U1-7P038, Class 1 Piping Weld, RC-04-PSF-1001-04 Datasheet 94U1-451P040 Dye penetrant inspection revealed two indications in the welded area that were a result of the welding process. 1.0 INTRODUCTION

From May 6,1993 to May 12,1994 repairs and replacements were performed on item:: at PBNP Unit 1. These repairs and replacements were completed in the first examination outage of the second period of the third 10-year interval at PBNP Unit 1. 2.0 ABSTRACT OF REPAIRS / REPLACEMENTS f 2.1 Repairs / replacements performed on ASME Class 1 and 2 items are documented on the attached NIS-2 forms. Repairs / replacements on ASME Class 3 items are documented on NIS-2 forms and maintained on file at PBNP. They are not included i in this report in accordance with ASME Section XI, IWA-6210(c). 2.2 The following is an abstract of the repairs / replacements performed at PBNP Unit 1 during first examination outage of the second period of the third 10-year interval. l 2.2.1 Atmospheric steam dump valve (MS-02016) had the trim cage lock assembly removed. A small crack was buffed out where the cage lock assembly was welded to the valve body. A new trim assembly was welded back in place. This work is documented in MWRs 932110 and 936512 and RRMs 93-0032 and 93-0086. 2.2.2 The bolting on the flange connection of flow transmitter FIT-930 was replaced because of corrosion from boric acid leaking from the fiange. This replacement is documented in MWR 932936 and RRM 93-0039. 2.2.3 Five Class 2 supports were modified in accordance with the requirements of the NRC IEB 79-14 reconciliation program. The modifications included shiming the lateral clearances to ensure the supports meet their existing design requirements. This work is documented in RRM 93-0075 and MWRs 935382,935383,935385,935389, and 935391. 2.2.4 The disk assemblies in valves SI-854A and SI-854B were replaced as part - of the check valve upgrade per MR 90-260. This work is documented in MWRs 936352 and 936355 and RRM 93-0078. 2.2.5 A linear indication was identified in the SI piping system. The indication was caused by work on an adjacent support. This was buffed out. This work is documented in MWR 936530 and RRM 93-0085.

This replacement is documented in MWRs 935950,940285, and 940232 and RRM 94-003. 2.2.7 The supporting steel for a spring support EB-1-MS-H5 on the main steam piping was relocated. This replacement is documented in MWR 940364 and RRM 94-0004. 2.2.8 Pipe support H-10 was modified in accordance with MR 93-047 to upgrade for the NRC IEB 79-14 reconciliation program. This work is documented in MWR 940365 and RRM 94-0005. 2.2.9 The pipe clamp on support SI-04 was replaced with a heavy duty clamp. This replacement is documented in MWR 940369 and RRM 94-0006. 2.2.10 Shims and anchorage were installed on supports SI-10 and R-194 to upgrade the support for the NRC IEB 79-14 reconciliation program. This replacement is documented in MWRs 940371 and 940373 and RRM 94-0007. 2.2.11 Test connections on the main feed system were installed. These test connections facilitated the testing of main feed check valves CS-466AA/BB j and CS-476AA/BB. This replacement is documented in MWRs 940293, 940294,940461, and 940463 and RRM 94-0008. i 2.2.12 The support on valves SI-839A, B, C, and D was reinforced by adding weld and plate material. This work is part of MR 93-047, NRC IEB 79-14 reconciliation program. This work is documented. in MWR 940370 and RRM 94-0010. 4 2.2.13 Shims were added to support R102 as part of the NRC IEB 79-14 reconciliation program. This work is documented in MWR 940372 and RRM 94-0011. 2.2.14 RCP seal water and excess letdown piping supports PS CH 151R-2-H222, PS CH 151R-2-SKA-1 and PS CH 151R-SKA2 were modified in accordance with the requirements of the NRC IEB 79-14 reconciliation program. This work is documented in MWRs 941065,940367, and 940368 and RRM 94-0012. 2.2.15 The U-bolt was removed from piping support SK-2 in accordance with the requirements of the NRC IEB 79-14 reconciliation program. This work is documented in MWR 940801 and RRM 94-0015.

This work is documented in MWR 935020 and RRM 94-0019. 2.2.18 The volume control tank relief valve (CV-257) was designed to relieve a water environment. This valve was replaced with a valve designed to relieve an H and N atmosphere. This replacement is documented in 2 2 MWR 915279 and RRM 94-0020. 2.2.19 MS-2006 was sent to the CRANE-ALOYCO facility for repairs as part of the relief valve program. This repair is documented in Purchase Order 247390 and RRM 94-0021. 2.2.20 MS-2008 was sent to the CRANE-ALOYCO facility for repairs as part of the relief valve program. This repair is documented in Purchase Order 247390 and RRM 94-0022. 2.2.21 The bonnet and diaphragm assembly on CV-216 was replaced. This replacement is documented in MWR 913462 and RRM 94-0024. 2.2.22 The bonnet and diaphragm assembly on CV-212 was replaced. This replacement is documented in MWR 913461 and RRM 94-0025. 2.2.23 The base plate was shimmed and the grout pad was repaired on support MSH-01 in accordance with the NRC IEB 79-14 reconciliation program. This replacement is documented in MWR 940363 and RRM 94-0026. 2.2.24 The main feedwater check valves CS-466AA and CS-476AA were replaced because of leak by. This replacement is documented in MWRs 9401539 and 9401540 and in RRM 94-0027. 2.2.25 Analysis of the oil on residual heat removal pump P-010B showed a high heat problem in the bearings. The rotating assembly of the pump was replaced. This work is documented in MWR 9401614 and RRM 94-0029.

This work is documented in MWR 9401938 and RRM 94-0030. 2.2.27 During gasket replacement on the pressurizer manway, one of the manway bolts became galled (bolt No. 7). This bolt was replaced with one from stock. This replacement is documented in MWR 9401615 and RRM 94-0031. 2.2.28 The supports on valves SI-844A and SI-844B were modified to provide seismic restraint. This replacement is documented in MWRs 9401518 and 9401519 and RRM 94-0032. 2.2.29 Boric acid on the studs of RC-427 was noted during a system walkdown. The stud nuts were removed and replaced because of general corrosion from the boric acid. This replacement is documented in MWR 9401366 and RRM 94-0033. 2.2.30 Boric acid on the studs of the flan;;e connection for FIC-179 was noted during a system walkdown. The stud nuts were removed and replaced because of general corrosion from the boric acid. This replacement is documented in MWR 9401371 and RRM 94-0034. 2.2.31 Two support nuts on the tension bar were found loosened on support H-313. The nuts were tightened.
