Dear Mr. DeYoung:
The Contractor Equipment Business Operations of the General Electric Company detemined on this date that a reportable defect exists on a Vitro Laboratories 125-volt DC battery monitor supplied through us for use at the Wolf Creek Nuclear Generating Station. This is the only unit supplied by General Electric which may have this defect. The following is the text of our notification for your use as necessary:
The General Electric Company has been informed by Vitro Laboratories that a battery monitor purchased from them for use in Class 1E equipment at the Wolf Creek Nuclear Generating Station may have been assembled without an insulating washer on a stud-mounted Zener diode. The absense of this washer will cause the battery monitor to issue a " Battery Failure" indication and trip the circuit breaker through which the battery power is being supplied.
The General Electric Company has initiated action to have this unit returned for inspection and/or repair by our supplier.
It is estimated that the recycling of this-unit will take approximately one month.
