is to confirm our verbal notification of your Mr. Ralph This report Architzel of an inadvertent opening of a Power Operated Relief Valve (PORV).

At 2014 on April 26, 1983, while in Mode 5 at about 198'F in the Reactor Coolant System (RCS), primary system pressure was being With RCS pressure indicating approximately raised to about 400 psig.

400 psig on the plant computer and approximately 430 psig on was immediately isolated by shutting PIC-103-1, ERV-404 opened.

It its upstream valve (T.S. 3.4.9.3).

While in T.S. action statement 3.4.9.3.a (a), RCS temperature since shutdown cooling increased to greater than 2OO"F (Mode 4),

flow had been terminated and one RCP was running per T.S. 3.4.1.3.

While troubleshooting the problem with the PORV, at 2150, a stort caused a loss of control power to ERV-402, rendering the second PORV Control power was restored to ERV-402 (T.S. 3.4.9.3).

inoperableBy 2230, both PORVs were in service in MPT enable.
