Technical Specification 6.8.1 requires that written procedures shall be established, implemented and maintained that meet or exceed the requirements of Regulatory Guide (RG) 1.33, including administrative procedures.
Station Procedure No. 119.5, " Loose Equipment Storage," provides the requirements for anchoring and storing loose equipment in the Reactor Building (RB) and Turbine Building (TB).
Contrary to the above, during the inspection period for April 19, 1994, through May 30, 1994, multiple examples of improperly anchored and stored loose equipment were identified both in the RB and TB.
This is a Severity Level IV violation (Supplement 1).
ResDonsql GPUN concurs with the violation as stated.
The cause for this violation was a programmatic weakness in the implementation of the Oyster Creek 119.5 procedure for loose equipment control.
Corrective actions were immediately initiated by sending out teams to secure or restrain all identified instances of improperly anchored and stored loose equipment.
[0RRECTIVE STEPS THAT WILL BE TAKEN TO AVOID FURTHER VIOLATIONS:
The Maintenance Director will issue a memo to the entire Maintenance staff outlining Oyster Creek's failure to comply with the requirements of OC Station Procedure No. 119.5, " Loose Equipment Storage" thus constituting a violation of technical specification requirements.
Additionally, all Maintenance personnel will be sensitized as to their accountability and responsibility to comply with the subject procedure.
Other applicable departments on site will issue a similar memo to their staffs.
The above memo will be reviewed at all Maintenance crew meetings so that the message is transmitted down to the work force.
The members of the Management Observation team and the Off-Shift Tour Program will be re-sensitized as to the requirements of Procedure 119.5.
DATE FOR FULL COMPLIANCE:
August 31, 1994 when the above noted corrective actions are completed.
