PVNGS UNIT 1 I.
DESCRIPTION OF DEFICIENCY Bechtel Nonconformance Report N-A-558 was initiated to correct unacceptable welds on the partially assembled Unit 1 Gas Stripper supplied by Combustion Engineering. This item was delivered partially assembled to support the Unit 1 construction schedule.
The defective welds were on a 2" pup-piece between a column nozzle and 4" to 3" reducer; however, when the N-5 Code Data Report was checked, it was discovered that the pup-piece was not accounted for as material meeting the requirements of the AS!!E Code.
A subsequent review of this condition by Bechtel site engineering found that the pup piece was to be replaced as part of the normal installation procedure. Helix Process Systems' Field Assembly Procedure for the Unit 1 Gas Stripper, Specification No. A3979607, Section 4.2.1.5, requires replacement of the pup piece during field assembly of this unit.
The Bechtel JCR and subsequent DER were inadvertently initiated by Bechtel field personnel not having knowledge of the existence of the installation procedure for this particular piece of equipment.
CE's supplier, Helix Process Systems, prepared the procedure uniquely for the Unit 1 Gas Stripper and provided the document with shipment of the equipment. Since Behtel's Drawing and Data Control system was by-passed, the personnel responsible for reviewing these reports were not aware that a unique procedure existed.
II.
ANALYSIS OF SAFETY It!PLICATIONS This condition is evaluated as Not Reportable. A safety significant condition never existed. Although the above condition reflects an oversight in Bechtel's preliminary investigation, this event does not constitute a breakdown of the Quality Assurance Program as defined by 10CFR50.55(e).
III. CORRECTIVE ACTION The NCR will be dispositioned to replace the piece with material meeting the requirements of the ASIE Code as per the Field Assembly Procedure, which has been retained in Bechtel's Drawing and Data Control system for record purposes.
e-e Action to preclude recurrence is being taken by notifying Combustion Engineering that all assembly and/or instruction manuals, including special case conditions such as the Unit 1 Gas Stripper, must be submitted to Bechtel Engineering prior to delivery of the equipment.
By providing this information to Bechtel Engineering, the proper control and distribution of this information can be accomplished in the same manner as the other CE design documents.
This condition is isolated to the CE/Bechtel interface for NSSS items, because the remainder of the Project suppliers are required to use Supplier Deviation Disposition Requests for conditions such as this.
The Gas Strippers for Units 2 and 3 are being delivered to the jobsite fully assembled, therefore no further repairs or replacement will be necessary. _.
