ENCLOSURE SPECIAL REPORT 91-02 UNIT 2 DESCRIPTION OF EVENT i
On March 14, 1991, with Units 1 and 2 operating in Mode 1 (100 percent power, 2235 psig, and 578 degrees Ferhenheit), it was determined that fire barrier door A-44 would be nonfunctional for a period of greater than seven days.
Fire door A-44 is on Elevation 669 of the auxiliary building, serving the Unit 2 penetration room. On March 8, 1991, the grout above door A-44 was observed to be cracked and loose. A work request (WR) was initiated and a breach permit was posted.
Limiting Condition for Operation (LCO) 3.7.12 was entered at 1335 Eastern standard time (EST). The operability of the fire detectors on one side of the fire barrier was verified and an hourly, fire watch patrol was established as required by LCO 3.7.12 Action-Statement (a).
The roving fire watch remained in effect until the fire door was returned to service.
Subsequently, the grout was replacedt the door was returned to operable status on March 29, 19914 and LCO 3.7.12 was exited at.435 EST.

CAUSE OF EVENT Fire door A-44 is an 8-foot wide, steel, double door.
The ;f fact of the heavy door closing against the steel f rame over a period of time c,mbined with normal aging of the grout, may have contributed to the as-found condition of the grout. The closing mechanism on the door was checked, and the force to automatically close the door was not f ound to be excessive.
The cause of the fire door not being returned to functional' status within the actual time required to accomplish the repair work.
The WR was given a_P-3.
seven-day timeframe was because of the priority placed on the WR and the priority in accordance with Plant Manager's 3tandard Procedure (PMSP) 6.2.1,
" Maintenance Management System Initiation of Work Request " which governs the-i initiation and planning process for maintenance work.
The P-3 priority requires the activity to be planned and the work started within seven days of the initiation of the WR.
The WR was. initiated on March 8. 1991, and sent to the craf t on March 14, 1991,-for work after completion of the planning.
The craf t fcrenmn recognized that the ' time required to complete the work activity would be greater than the one day remaining for.the-LCO action.
ANALYSIS OF EVENT This event is being reported in accordance with the requirements of Action Statement (a) of LCO 3.7.12.
A-roving fire watch was established to inspect.
the affected area on an hourly basis.
The existing fire detection and suppression system for the affected area was-determined to be operable and would actuate in the event of a fire. Therefore there was no danger to plant personnel or equipment,

CORRECTIVE ACTION The grout was replacedt a postmaintenance test (PMT) was performed on the doort and the door was returned to operable status on March 29, 1991.
To promote mare timely return to service. PMSP-6.2.1 will be revised by May 3, 1991, to priorittee WRs associated with a neven-day LCO action requirement as P-2 (immediate Attention).
A P-2 WR must be picnned within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> and work started as soon as possible.
COMMITMENT PMSP 6.2.1 will be revised by May 3,1991, to prioritise all firo protection WRs associated with a seven-day LCO action reqsirement as P-2 (Immediate Attention).
