MAJOR SAFETY-RELATED MAINTENANCE NOVEMBER,1982 M-1 Steam generator No. 4 power operated relief valve,- MRV-243, had a body-to-bonnet leak. Disassembled and cleaned the valve. Re-assembled using new gaskets and had the valve tested.

M-2 No.1 steam generator blowdown sample containment isolation valve, DCR-301, was leaking by. Replaced the valve and had the new valve tested.

M-3 Following disassembly of the East centrifugal charging pump, non-destructive examination identified several indications on the i

internal cladding of the pump casing at the suction nozzle. The indications were removed by grinding to a maximum depth of 3/32".

NDE fellowing excavation revealed no further indications.

C&I-l Emergency boration flow indicator, QFI-420, was inoperable. The problem was traced to a defective 80 VDC power supply diode solder i

connection on the indication loop E to I converter. Repair of the solder connection and recalibration of the loop returned the indicator to operability.

C&I-2 Upper containment volume sampling isolation valve, ECR-21, would not open. A defective "asco" solenoid valve coil was replaced, and the actuator piston o-rings were changed to provide proper operation of ECR-21.

4 C&I-3 Steam generator No. I sample isolation valve, DCR-301, was leaking by. The valve operator was removed and replaced and the valve stroke was reset.

Proper operation was verified.

C&I-4 Volume control tank valve to vent header asco solenoid actuator, RS0-300, was not cycling the vent valve. The asco valve diaphragm 4

was discovered to have several perforations in it. An overhaul kit was installed to replace the diaphragm. Correct valve op-eration was verified after repairs.

C&I-5~

Source range nuclear instrumentation channel, N32, was indicating ten (10) counts with no power applied to the associated electronics i

drawer. The detector cable connector on the preamplifier was re-placed to eliminate the spurious reading.
