, Unit 1 - Cold shutdown for refueling Unit 2 - at 1045 We Unit 3 - at 1091 WE Identification and Descriotion of Occurrence Performance of Surveillance Instruction'4.7.A.2.g-3 revealed that the leakage rate for "A" line MSIV's was a total of 4883.3806 SCFH. Using conservative assumptions, the leak rate applicable to each valve is 2441.6903 SCFH. This causes the total type B and C tests to exceed 60% La or 655.9 ' SCFH (T. S. 4.7 a.2.g) and causes the technical specification limit of 11.5 SCFH (T. S. 4.7.a.2.1) to be exceeded.

Annarent Cause of occurrence Leakage past the main valve seat.
