Metallurgical defects in motor pinion gear. identified February 1.7,1983.

INITIAL REPORT:

Initial report was made to Mr. A.'Ignatonis, Region II NRC, on March 18, 1983 by Messrs. W. O. Henry.and J. K. Berry, Duke Power Company.

DESCRIPTION'0F' DEFICIENCY:

Motor pinion gear on.Limitorque actuator serial number 249637, Westinghouse valve serial number 10000GM88FME01D04S740002, has metallurgical defects in the form of cracks and porosity in two gear teeth. Valve is a ten inch gate valve, Duke. item number 9J-209, tag number 1NIO88B. This active valve was supplied per Duke Power Desig'n Specification CNS-1205.00-0008 and Mill. Power Order A-98524. The valve is a low pressure accumulator tank discharge isolation valve.

' ANALYSIS OF SAFETY IMPLICATIONS:

Failure of motor pinion gear could preclude this active valve completing its safety function.

CORRECTIVE' ACTION:

Defective gear has been replaced. Duke considers this defect to be an isolated case.
