ITEM A Technical Specification 6.11 requires that procedures for personnel Radiation Protection be prepared and adhered to for all operations involving personnel radiation exposure.
Radiation Protection Procedure RF-2,_ Revision 1, Radiation Work Permit Procedure, Section 5, requires the RWP 1cadman to ensure that all RWP instruc-tions are followed.
RWP No. 8415, General Clean-Up of East and West Instrument Rooms, dated May 18, 1982, required that rubber gloves, a cap, and taped coveralls be worn.
Contrary to the above, on May 18, 1982, a 1cadman, working under RWP No. 8415, did not ensure that all RWP instructions were followed.
The leadman and his co-worker wore only shoecovers and cotton glove liners as protective clothing.

RESPONSE
The following actions were taken at de time of de violation:
1.
Peuonnet involved were surveyed to de,termine if dere was any external contamina. tion involved. None was found.
2.
Nasal smears were taken to determine if any contamina.ted materlats were inhated.
Results were <100 dpm/ smear.
3.
A meeting was held with flie personnet involved, neir supervisors, and tite Radiation Protection Supervisor to ensure dat personnet involved btew de importance of fottowing RWP rules.
In addition, ue following steps were.taken to avoid further violations:
1.
Disciplinary action has been taken in the form of a written reprimand to tlie peuonnel involved.
2.
The policy requiring automatic disciplinary action for violations of RWPs was emphasized by memo from de Gene >utt Superintendent, Nucleat Generation.
In view of de above, Niagara Mohawk Power Corp. is in compliance.
