Docket tjo. 50-361 Prompt Report Licensee Event Report tJo.099 San Onofre tJuclear Generating Station, Unit 2 Pursuant to Technical Specification 6.9.1.12b to Operating License tJPF-10 for San Onof re Unit 2, this letter confirms our September 13, 1982, pr ompt notification to the tJRC of a reportable occurrence involving Limiting Condition for Operations (LCO's) 3.7.5 and 3.0.3 on the Control Room Emergency Air Cleanup System (CREACUS).

On September 13, 1982 at 1230 while in Mode 2 both trains of CREACUS failed to start. Initial investigation revealed that Train A failed to start because the kirk key on breaker 2A0615 of emergency air-conditioning unit E-418 was positioned halfway toward its normally closed position, consequently Train A could not be energized. Operability of the Train was established by repositioning the kirk key in the breaker to its locked closed position at 1302. Investigation into the cause of the failure of Train B is continuing. The initial review indicator that the damper 2itV9703 associated with air-conditioning unit E-419, was interlocked such that i t prevented the f an f rom starting. The Train was restored to operable status at 1350.
