Keppler Regional Administrator, Region III Office of Inspection and Enforcement U.S. Nuclear Regulatory Commission 799 Roosevelt Road Glen Ellyn, Illinois 60137 RE: Perry Nuclear Power Plant Docket Nos. 50-440; 50-441 Hilti Kwik Bolt Adjacent Installation Epacing Requirements

[RDC 66(83)]

Dear Mr. Keppler:

This letter serves as an interim report pursuant to 10CFR50.55(e) concerning violation of specification requirements during installation of adjacent Hilti Kwik Bolts. Initial notification that this problem is being evaluated was made to Mr. F. Jablonski of your of fice on February 7,1983, by Mr. J. Kerr for The Cleveland Electric Illuminating Company.

This report contains a description of the deficiency, the corrective action taken to date, and describes the method planned to complete our evaluation.

Description of Deficiency During field inspections of some installed Hilti Kwik Bolts, it was determined that the minimum spacing requirements established by the installntion attachment specifications, SP108/208, have not been met in all cases. This condition is documented on Nonconformance Report CQC 2644 and involves the spacing between adjacent Hilti Bolts, grouted-in bolts and the free edge of concrete members.

Corrective Action Taken To Date All safety-related contractors installing Hilti Bolts were informed by the Project Organization by letter dated January 28, 1983, of the proper inter-pretation of spacing requirements from installation specifications SP-108/208, which is to be applied for all future work. In addition, specifications SP-108/

208, as attached to all construction specifications, are in the process of being revised by issuance of Engineering Change Notices to explicitly clarify the requirements for bolt spacing and edge distances. For all future work, the affected contractors have been informed that any apparent conflict between drawing requirements and the spacing requirements of SP-108/208 shall be brought to the attention of Project Organization Engineering for resolution prior to installation.

Completion of Evaluation -

On-site load.teets of multiple, closely spaced Hilti Kwik Bolts installed in

, typical job placed concrete will be conducted in.an attempt to re-evaluate the conservative existing spacing criteria contained in specifications SP-108/208. ,

The results of'these tests will form the basis for the program to evaluate the work previously installed. If favorable test results are obtained, specifica-tions SP-108/208 will also be revised - te reflect the spacing requirements for future installations. At th?s time, it is anticipated that our next report on

'this subject will be submitteo by July 1,1983.

Please call if there are additional questions.

Sincerely, Murray R. delman Vice President
