DESCRIPTION OF DEFICIENCY As documented by the attached Cooper Energy Services 10CFR Part 21 Report dated September 30, 1982, a diesel generator engine, serial
- 7188 (Project Tag No. 3-!!-DGA-H01), has been discovered to have been delivered with a defective piston in cylinder #7.
II.
ANALYSIS OF SAFETY IMPLICATION This condition is evaluated as Reportable since, if left uncorrected, the condition could preclude the diesel generator from performing its safety-related function.
III. CORRECTIVE ACTION Nonconformance Report IfG-1445 has been issued to control and document the replacement of the piston by Cooper Energy Services prior to the start-up date for the unit.
