Dear Mr. Keppler:
This letter is to confirm our telephone call made to Mr.
E.R. Swanson, NRC Region III Resident Inspector, at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br /> on December 1 1982, in accordance with Unit 2 Tech-nical Specification 6.9.1.8 to inform him of the following event.
On December 1, 1982, the Unit 2 West Centrifugal Charging Pump (CCP) was rendered inoperable following a modification performed on the East and West Centrifugal Charging Pump's common mini-flow line.
Unit 2 was in Mode 5 at the time and the West Centrifugal Charging Pump was the operable charging pump pursuant to Technical Specification 3.1.2.3.
The scheduled work was to affect only the East Centrifugal Charging Pump and the mini-flow line of the West Centrifugal Charging Pump.
The cut was to be made downstream of valve CS-298W which was the isolation point for the West Centri-fugal Charging Pump.
The actual cut, however, was made upstream of valve CS-298W which is a pressurized portion of the West Centrifugal Charging Pump outlet piping.
This rendered the West Centrifugal Charging Pump inoperable and placed the Plant into Action Statement A of Technical Spe-cification 3.1.2.3.
Our investigation into this matter is in progress and a followup report will be submitted by December 15, 1982.
