ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 1, 2 & 3 I.

DESCRIPTION OF DEFICIENCY liigh Voltage Cubicles 1, 2, 3-J-DGAB-B03 supplied by Cooper Energy Services are seisnic category I per specification 10407-13-MM-018.

Vendor installation drawing imi-018-209 shows anchor bolt holes on-three (3) sides of the cubicle. Cubicles were qualified by a shake test on one of the cubicles.

The Units 2 and 3 liigh Voltage Cubicles have anchor bolt holes on three (3) sides as per referenced drawing; however, the Unit 1 N/

panels have anchor bolt holes on four (4) sides. Vendor confirmed in telecon, and confirmed in writing, that a Unit 1 cubicle was used for the qualification test using anchor bolt holes on four (4) sides. Units 1, 2 and 3 cubicle foundations were designed'using the vendor drawing, which is not in agreement with the qualification mounting; therefore, the cubicles as installed'are not seismically qualified.

This condition was inadvertently found during an engineering walkdown of the diesel generator installation, and subsequent telecon with supplier. The qualification documentation submitted to Bechtel did not specify the particular anchorage used for the shake test.

II.

ANALYSIS OF SAFETY IMPLICATION This condition is evaluated as reportable. The cubicles as Installed are not, seismically qualified and therefore nay~not be capable of perforning their safety related function in the event of a Safe-Shutdown Earthquake.

'l III. CORRECTIVE ACTION h

2L For Unit 1, Design Change Package 1SH-DG-022 will be issued to

provide an acceptable anchorage on four (4) sides. This carrection will be developed first on Unit 2.

For Unit 2, NCR ?tG-1112 and FCR will be used to implement a repair which provides the required anchorage. For Unit 3, Cooper will provide mounting holes for anchorage on four (4) sides. Tha Bechtel foundation' design will'be,.

updated to accommodate this correction.

In addition to reportability under 10CFR50.'55(e), PVNGS Project- !

considers the deficiency to be rep 3rtable under the requirementsiof 10CFR Part 21 by the supplier. Deficiency Evaluation Report 82-38' '

addresses the reporting requirements specified under 10CFR
