This is to confirm a telephone call made at 1435 hours0.0166 days <br />0.399 hours <br />0.00237 weeks <br />5.460175e-4 months <br /> on November 23, 1982, to Mr.
N.E.
DuBry, NRC Region III Re-sident Inspector in accordance with Technical Specification 6.9.1.8.b.
On November 23, 1982, during a routine surveillance test of the Unit 1 Essential Service Water System (West Train),
motor operated valve WMO-753 would not open as required.
Further investigation determined that t.he valve was opera-ting in a reversed manner due to a Design Change which was performed on November 17, 1982, at the valves Motor Control Center.
The Design Change authorized the replacement of the existing cable which fed the Motor Control Center with a larger cable.
During the course of the installation, the leads of the new cable were reversed at the Motor Control Center.
The error was not discovered at the time and the Motor Control Center was returned to service.
The Motor Control Center also feeds a Turbine Driven Auxiliary Feed Pump Room exhaust fan.
Upon discovery on November 23, 1982, the problem was cor-rected and the associated equipment was restored to normal.
The requirements of Technical Specification 3.7.1.2 were not M
met due to the nature of the event and method of discovery.
