Malfunction of a Safety Related Nuclear Valve A Masonellan Servite Engineer, responding to a call from Union Electric, Callaway Creek Station, found two valves of a group of four to be inoperative.

The valves are 2" Main Steam Header Warm-up Valves supplied to Bechtel Corporation on Bechtel Order Number 10466-J-601-A, Tag Number 0-AB-HV-12, 15, 18 and 21. The valves, Model 38-40421, were manufactured by Masonellan, at the Norwood, Massachusetts facility, on Masoneilan's Work Orders N-00172-17, N-00172-18, N-00173-17 and N-00173-18. Four valves are installed at the Callaway Creek Station of Union Electric and four at Wolf Creek Station of Kansas Gas and Electric. The defect is limited to the 8 valves at these two stations.

The nature of the defect is a machining error in the valve's main plug allowing a Retaining Ring to shift sideways permitting the Pilot to lift and lock in position preventing tight shut-off of the valve.

Masonellan related the defect as causing a malfunction of a safety related valve on October 27, 1982.

The errant part, Plug No. 040001-285-781 was manufactured in a lot of 8 pieces solely used on the above listed valves. We are uncertain if all eight have the defect, but are confident it is limited to this manufacturing lot.

Masonellan is immediately manufacturing 8 replacement plugs, to cover all valves in question, at the Norwood Massachusetts manufacturing facility. The replacement parts will be sent to the sites by November 24, 1982.

This report and our remedial actions are in compliance with Federal Regulation 10CFR21.
