Dear Sir:
The NRC was notified of a potentially reportable deficiency in Reference (A) and an Interim Report was transmitted by Reference (B). At that time, it was estimated that a Final Report would be available by November 5, 1982.
Due to the extensive investigation and evaluation required, a revised Interim Report is attached.
It is now expected that this information will be finalized by January 12, 1983, at which time a complete report will be submitted.

- INTERIM REPORT - REVISION 1 - DER 82-38 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT 3 I.
POTENTIAL PROBLEM During receiving inspection four (4) 8" butterfly valves supplied by Henry Pratt were received at the jobsite with loose Limitorque actuator to gear box mounting bolts. These bolts are initially secured by Limitorque and not loosened by Pratt during assembly to the valve.
One each bolt from two valve actuators were found not installed and lying on the shipping pallet.
Contact with Limitorque disclosed that a specific torque value was not specified for the four (4) hex head bolts securing the SMB to HBC unit; however, per Limitorque telex dated 6/3/82, the bolts should be tightened to approximately 85 ft. lbs. or as tight as reasonably possible. The valves are the containment purge system isolation valves.
II.
APPROACH TO AND STATUS OF PROPOSED RESOLUTION This deficiency report has been transmitted to Henry Pratt for review and evaluation. Also, Bechtel's Snpplier Quality group is investigating this item to determine the cause. Bechtel Quality Assurance will perform a surveillance activity at the jobsite to determine the extent of this condition.
III.
PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by January 12, 1983.
