Dear Mr. Keppler:

On August 18, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During chipping to repair spalled concrete near an embedded plate in the-primary shield wall workers discovered two deformed wire anchors missing from the plate.

Further investigation of similiar plates revealed additional missing anchor wires as well as anchors bent beyond acceptable limits, cracked welds between plate and anchor, and nicked anchor wires. These plates had been installed prior to cessation of safety related work in August, 1979.

PSI is presently evaluating techniques which will allow it to examine the condition of deformed wire anchors on embedded plates installed prior to August, 1979. The results of this examination will determine what corrective action, if any, is necessary.

This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report. will be filed
