Dear Mr. Keppler:

On June 10, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). While refurbishing structural steel, it was discovered that several pieces of structural steel had been incorractly fabricated. The deficiencies involved were out of tolerance edge distance, spacing, and size of bolt holes and excessive weld undercut.

The out of tolerance deficiencies had not been discovered previously because they were concealed by connection attachments (clip angles) which were removed during refurbishing. The excessive weld undercut deficiencies were not a previous inspection attribute.

It was determined that bolt hole size, spacing and edge distance are only of concern for sliding type connections and not friction type connections. The steel erector, Newberg-Marble Hill, has inspected and, if necessary, repaired the structural steel in question prior to its erection in accordance with their procedures WPN 5, WPN 5.02, WPN 26, WPN 26.01 and associated quality control procedures. Also, similar structural steel members previously erected have been inspected and, if necessary, repaired in accordance with the aforementioned procedures.

This letter is intended to fulfill the requirements of a final report as defined in 10 CFR 50.55(e).
