Special Report SR-94-02 Unit 2 Vibration & Loose Parts Monitor operability of channel 0 i

Associated Tech spec 3.3.3.10 Related PIP's:

PIP 2-M94-0362 Cause of Malfunction:

Channel 8 (2MIMT5170, S/G 2D) of the i

Vibration & Loooo Parts Monitor (V& LPM) was declared inoperable af ter it was discovered to have signal problems by Operations on February 14, 1994.

WO 94012881 was initiated and channel investigated by IAE.

Signal problems were verified.

The cause of the low noise level is associated with sensor or poor cable terminations in RX2.

j Instrument Vendor:

V&LPH manufactured by Rockwell International Generic Concerns:

The V& LPM has a history of unreliable performance. Specifically, numerous sensor cable and termination problems have been experienced due to their susceptibility to damage during. installation or nearby maintenance activities.

This channel was also declared inoperable on 4/14/93 and repaired during 2EOC8.

Refer to Special a

Report SR 93-05 for additionaly information.

Compensatory Measures:

No compensatory measures are planned or required.

Plans for Restoring Operability:

Channel 8 of the V& LPM will be repaired under WO 94012881 during the next Unit 2 refueling outage (2EOC9),

or during installation of NSM MG-22096, when cables and terminations are accessable.

Future Plans for V& LPM:

NSM's MG-12096 (Unit 1) and MG-22096 (Unit

2) are scheduled for completion during lEOC9 and
2EOC9, respectively.

Implementation on these NSM's began during lEOC8 and 2EOC8, respectively. These NSM's will install a completely new system and address system failure trends.
