Dear Sir:

In response to the violation which you reported in Reference (b), PSNH offers the following:

NRC Notice of Violation (443/82-13-01):

As a result of the inspection conducted on October 1 - November 5, 1982, and in accordance with the NRC Enforcement Policy (10CFR2, Appendix C) published in the Federal Register on March 9, 1982 ( 7FR9987), the following violation was identified:

10CFR50, Appendix B, Criterion X ant the Seabrook Station PSAR require that inspection shall be provided fe activities which affect quality.

Pullman Power Products Pipe Support Procedure JS-1X-6 provides installation inspection requirements including hold points for safety-related expansion anchors. An associated Pullman Procedure VI-5 prohibits work from proceeding past an inspection hold point until the item has been inspected and signed off.

Contrary to the above, on October 28, 1982, work on the expansion anchor installations for Pipe Support 1212-SH-06 had proceeded past six designated inspection hold points without the required inspection and sign off.

This is a Severity Level V Violation (Supplement II).

Corrective Action Taken and Results Achieved:

The piping contractor, Pullman-Higgins, has revised the expansion anchor process sheet in order to make applicability of the witness and hold points clear to craft and inspection personnel. Witness points shall be verified by the QC inspector at any point in the installation process, while hold points require QC approval prior to proceeding to the next operation. Unacceptable initial torque shall be cause for rejection and will require a new process sheet in order to retorque the installation.

Pullman-Higgins issued Field Instruction 177 to explain the use of the new process sheets and to effect their use.

Previously released field erection packages which contain the obsolete form will be returned to the hanger department for issuance of the revised process sheet.

Corrective action was completed on November 10, 1982.
