Dear Mr. Keppler:

This is to confirm our verbal report to Mr. N. Dubry, NRC Region III Inspector, on December 30, 1982, at approximately 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br />, at which time we informed Mr. Dubry of the location and nature of an obstruction in Unit #2 valve SI-152-S, which prevented attainment of the minimum required flow through the south safety injection header. The presence of a flow obstruction was indicated on December 23, 1982, at which time performance testing showed a maximum attainable flow of 260 gpm (vs.

required flow of 300 gpm, minimum) through the flow path containing valve SI-152-S. Systematic investigation since that date resulted in discovery of the obstruction (the unattached disc from a previous repair of valve SI-152-S) lodged within the valve body.

An investigation into the cause of this event has been initiated. A complete report will be submitted in fourteen (14) days.

Subsequent to the above December 30, 1982 report, the obstruction was removed from valve SI-152-S and the correct SI flow rate was established with no further difficulties.
