Dear Sir:
On October 16, 1978, a report was made under the provisions of 10CFR50.55(e) concerning a possible deficiency in the three-quarter inch bolts used to attach T.echanical guard barriers to the sides of the high density fuel racks.
In accordance with the reporting requirements of 10CFR21, the following information is submitted:

Description of defect, deficiency, or failure to comply:
NUS Corporation has discovered during their 0A records check that Speedway Machine and Tool Company (SMATCO) does not have documented evidence of a mechanical properties test after the heat treatment of the 64 Item 7 hex head bolts on NUS drawing 5094M2000. These bolts are used to attach mechanical guard barriers to the sides of the high density spent fuel racks.

Corrective action which has been, is being or will be taken, the indi-vidual responsible and the length of time to complete the action:
NUS has requested the return of the 64 bolts to SMATCO. SMATCO will in turn arrange for testing of a sample from the bolt lot, or if that is impossible, replace the bolts. Vepco expects the problem to be resolved within four weeks.
