I.

Narrative Summary of Operating Experiences Limerick Unit 1 began the month of February at a nominal 70%

of rated thermal power (RTP) in end-of-cycle coastdown.

On February 4, 1994, at 0908 hours0.0105 days <br />0.252 hours <br />0.0015 weeks <br />3.45494e-4 months <br /> reactor power reduction

'i commenced for the Unit 1 fifth refueling outage (1R05).

At 1300 the mode switch was placed in shutdown to initiate a reactor scram and Unit 1 entered OPCON 3.

On February 28, 1994, at 0043 hours4.976852e-4 days <br />0.0119 hours <br />7.109788e-5 weeks <br />1.63615e-5 months <br /> Unit 1 entered OPCON 4 when the reactor head was fully tensioned.

Unit 1 ended this operating period in OPCON 4 in day 24 of 1R05.

II.

Challenges to Main Steam Safety Relief Valves There were no challenges to the Main Steam Safety Relief Valves during the month of February.

Narrative Summarv of Operatina Experiences Unit 2 began this operating period at 100% of rated thermal power (RTP).

On February 17, 1994, at 0926 hours0.0107 days <br />0.257 hours <br />0.00153 weeks <br />3.52343e-4 months <br /> power was reduced to 90%

RTP for main turbine valve testing.

Power was restored to 100% at 1548 hours0.0179 days <br />0.43 hours <br />0.00256 weeks <br />5.89014e-4 months <br />.

On February 16, 1994, at 1410 hours0.0163 days <br />0.392 hours <br />0.00233 weeks <br />5.36505e-4 months <br /> the 2A Reactor Recirc pump ranback to minimum speed during the restoration from Joactor enclosure ventilation testing.

In order to perform this testing the Reactor Recirc Motor-Generator (M-G) Set is locked in place.

Upon restoration the M-G Set was unlocked but a speed signal mismatch existed which resulted in the runback.

Procedures OT-104, Unexplained / Unexpected Reactivity Insertion, and GP-5, Power Operations, were entered.

Power was decreased to 35% in accordance with the reactor maneuvering instructions.

Power was restored to 100% RTP on February 17, 1994, at 0425 hours0.00492 days <br />0.118 hours <br />7.027116e-4 weeks <br />1.617125e-4 months <br />.

On February 18, 1994, at 0310 hours0.00359 days <br />0.0861 hours <br />5.125661e-4 weeks <br />1.17955e-4 months <br /> power was reduced to 95%

RTP for a control rod pattern adjustment.

Power was restored to 100% shortly thereafter.

On February 24, 1994, at 1335 hours0.0155 days <br />0.371 hours <br />0.00221 weeks <br />5.079675e-4 months <br /> a licensed operator inadvertently removed fuses from the 2B Reactor Recirc M-G Set circuit vice the 1B Reactor Recirc M-G Set circuit.

This resulted in the trip of the 2B Reactor Recirc Pump.

Procedure OT-112, Reactor Recirc Pump Trip, was entered and power was stabilized at 33% RTP.

At 1722 hours0.0199 days <br />0.478 hours <br />0.00285 weeks <br />6.55221e-4 months <br /> the 2B Reactor Recirc M-G Set was re-started.

Power was restored to 100% on February 25, 1994, at 0955 hours0.0111 days <br />0.265 hours <br />0.00158 weeks <br />3.633775e-4 months <br />.

On February 26, 1994, at 0937 hours0.0108 days <br />0.26 hours <br />0.00155 weeks <br />3.565285e-4 months <br /> power was reduced to 92% RTP for a control rod pattern adjustment.

Power was restored to 100% RTP shortly thereafter.

Unit 2 ended this operating period at 100% of RTP.

II.
