.e-y, SEQUOYAH NUCLEAR PLAh'T SPECIAL REPORT 82-3 UNIT 1 Event Description On July 31, 1982, fire detector 74F, in fire zone 70, was in the alarm state due to a steam leak in the spent resin storage tank area. The fire dLcector is located in the spent resin storage tank room which is a high radiation area making it inaccessible to maintenance personnel.
Since the alarm would not clear due to the steam leak, it was decided by plant maintenance to remove the entire zone from service; however, Operations personnel mistakenly thought only the detector was being disabled and not the entire zone. A temporary alteration control form (TACF) was issued to allow the fire zone to be removed from service.
On August 1, 1982, the steam leak was repaired, but the fire zone was not placed back in service. Since Operations personnel thought only the detector had been disabled, the TACF was lef t in place becatise engineering change notice (ECN) 5454 had been issued to remove fire detector 74F for ALARA considerations.
On September 9, 1982, during a review of TACFs, it was discovered that the fire zone had not been placed back in service. Upon discovery, the fire zone was placed back in service and no additional corrective action is planned. The removal of the fire detector will not violate the technical specifications since five detectors will remain in fire zone 70.
