ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1

'I.- DESCRIPTION OF DEFICIENCY During startup prerequisite testing of Target-Rock valves in the safety injection system, the Reed indicating switch clamp tack-weld was found to be either' broken or to break during positioning of the switch and tightening the adjustment nut. This condition causes loss of, or incorrect, valve position indication in the Control Room. lA total of six failures have been found, some of which had a broken bracket at the time the cover was removed for the positioning operation. Fourteen valves of this type,per unit are supplied by Cambustion Engineering as part of the NSSS contract as follows: SI-605, 606, 607, 608, 613, 623, 633, 643, 659, 600; CH-203, 205, 526; and RC403.

II. ANALYSIS OF SAFETY IMPLICATIONS APS and Bechtel requested assistance from. Combustion Engineering in evaluating andidispositioning,this condition, and concur with the analysis ~provided by the attached C-E letter V-CE-16593 that the condition is not reportable under the requirements of 10CFR50.55(e).

Should this condition remain uncorrected, it would not represent a safety significant condition.

.III. CORRECTIVE ACTION The Unit 1 NCR (SE-0487/0542) will be dispositioned to rework the six (6) broken brackets by rewelding the broken tackwelds. -Since the remainder of 'the material has been shipped, Units 2 and 3 indicating switches will be inspected and repaired on an as-requested basis. NCR's will be generated to document this corrective action.

'Ihe subject DER states that Reed Indicating Switches are experiencing structural failure during valve testing in the tack weld of the reed switch clamp in breaking loose during positioning of switch and tightening of the adjustment nut. A total of six switch failures have been found (SI 605, SI 607, SI 608, SI 613, SI 633).

his failure would not affect the operation of said valves. 'Ihe only effect would be a loss of position indication. The effect of this loss would be that the operator would lose the open/close indication for that valve. In a non-accident situation, and in the short-tem accident response, these valves are locked closed with power removed. Post-accident, the desire for position indication comes from tile need to vent the Safety Injection Tanks (SIT) when the RCS is being depressurized. This precludes the injection of nitrogen into the RCS. The preferred method for confirmation of the depressurization of the SITS is verification by the use of the IE-powered SIT pressure indication. Thus the loss of valve position indication is not significant. During a nomal plant cooldown the defect SITS is are not depressurized applicable. by drawing water instead of venting nitrogen so the subject This condition has been reviewedand, in C-E's opinion, is not reportable under the requirements of 10CFR50.55(e) or 10CFR Part 21.

A C-E evaluation has been performed in accordance with our Quality Assurance of Design Manual offices.as indicated in FAR 14273-468 which is available for review in our C-E Windsor ,
