DESCRIPTION OF DEFICIENCY j
Project procurement specification 13-CM-125B for miscellaneous Structural Steel awarded to Ametek Straza, paragraph 4.13.4 requires that "Al' weld procedures shall be submitted and approved prior to the start of fabrication... Welding procedures which conform with Sections 2, 3, 4'and 8 of the Structural Welding Code AWS Dl.1, shall be deemed as prequalified and are exempt from tests for qualification. Other procedures shall be qualified by tests as prescribed in Section 5 of AWS DI.l."
The reference drawing specifies a shop weld between ASTM A514 Grade F and ASTM A579 Grade 12 material for the NSSS "E" (Hot Leg Pipe) stops.
ASTM A579 is not a material listed under Section 8 of AWS DI.1, hence qualification tests are required.
A review of the welding procedures applicable to this order revealed that the weld between the above materials was made without the supplier submitting the required weld procedure to Bechtel for review prior to welding.
II.
ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as not reportable. Ametek Straza has submitted Supplier Deviation Disposition Request (SDDR) Log No. 3340 documenting that a non-qualified weld procedure AWS #1 was used. The SDDR also documents the Post Weld Qualification Test Records for AWS #1.
The acceptance of the SDDR was based on the satisfactory results of the tests. This information documents that a safety significant condition never existed and that no rework is required.
III. CORRECTIVE ACTION Bechtel has accepted SDDR Log No. 3340 dispositioned use-as-is by Ametek Straza, and has retained the weld procedut submittal in Drawing and
-Data Control. A review of all the documentacion related to this specification did not reveal any additional documentation deviations.
