Dear Sir:
The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an Interim Report is attached.
It is now expected that this information will be finalized by November 23, 1982, at which time a complete report will be submitted.

-e INTERIM REPORT - DER 82-41 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT 3 I.
POTENTIAL PROBLEM During testing activities in 4.16KV switchgear IE-PBA-S03 and IE-PBB-SO4, two conductors were inadvertently ~ pulled from the crimped area of the lug barrel. Further inspection revealed that some of the crimped lugs within the equipment do not exhibit identifying marks which identify correct use of crimping tools and dies. The AMP crimp lug terminations were inspected using requirements established by G.E. (dated May 14, 1982).
Random inspection of seven (7) compartments in Unit 3 switchgear at Control Building Elevation 100', found terminal block with conductors less than flush with the crimp connector barrel, and the end of the wire was not visible with magnification and high intensity light.
This condition is not indicative of open circuits but does not comply with visual criteria as stated by G.E.
II.
APPROACH TO AND STATUS OF PROPOSED RESOLUTION Bechtel Engineering is currently pursuing this matter with G.E. to establish appropriate field inspection and acceptance criteria for crimped terminations.
G.E. has been contacted to furnish their 1
factory criteria for crimped terminations which nueet Class IE requirements.
III.
PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT EvaF2ation of this condition and submittal of the Final Report is ic acast to be completed by November 23, 1982.
