Dear Mr. O'Reilly:

In accordance with 10CFR20.403b, the following event is reported. -

On April 16, 1982, at 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, a routine reading of a Contractor Health Physics Technician's TLD indicated an exposure of 6.6 rem. Backup dosimetry does not confirm this reading. This information was reported to Mr. C. Julian, NRC Region II, and Mr. B. J. Furr, Vice President Nuclear Operations, on the afternoon of April 17, 1982.

A facsimile was not sent because preliminary findings indicated the subject TLD had been previously exposed as part of a Quality Assurance Program and, -

as such, was not reportable pursuant to 10CFR20.403b. However, continuing investigation efforts indicate that the preliminary findings may not be accurate. Therefore, t'ils facsimile is submitted. Results from our investigation will be submitted in a thirty day report.
