f.4 p$g2' Carolina Power and Light Company (CP&L) hereby requests extended maintenance, for 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />, on the Residual Heat Removal (RHR) containment suction valve RHR-861A. His request is made pursuant to Technical Specification ('IS) 3.3.7.

  • On Augus t 11, 1982 a t approximately 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />, while performing PT-2.8B, RHR-861 A was noted to be opening only half way. RHR-861A was promptly declared inoperable. The plant wa s in a hot , zero power condition when the valve was declared inoperable. Subsequently a reactor trip placed the plant in hot shutdown at 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br />. The plant entered a 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> Limiting Condition for Operation (LCO) per TS 3.3.1.3, based on one RHR recircula tion flow path being inoperable at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />. Investiga tion into the cause of failure indicates that the valve disk may be deformed sufficiently to prevent full opening of the valve. The repair of this valve could potentially require more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> and, therefore, CP&L j requests an additional 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> before the plant would be required to i proceed to cold shutdown. This request is based on the fact that there is essentially no decay heat in the core, since the unit has not yet

! returned to power operation af ter the current six-month refueling outage, and an additional thermal cycle on the plant does not appear warranted.

Therefore, the plant can remain in the hot shutdown condition without affecting the public health and safety. In addition, if the plant were i to go to cold shutdown this would require operation of the RHR system, l one loop of which is inoperable due to isola ting RHR-861 A.
