This letter constitutes an interim report concerning the subject item, initially reported to the Commission as a potentially reportable deficiency under 10CFR50.55(e).
Radiographic access holes, adjacent to certain field butt welds in large bore piping are filled using threaded gamma plugs with seal welds on the outer wall of the pipe. At Zimmer, an estimated 420 plugs are utilized in essential piping systems.
The Quality Confirmation Program (QCP) documentation review and investigation found that certain plugs may be improperly installed.
Gamma plugs in reducer segments of piping systems and some plugs in the remaining piping spools may not meet the full thread engagement requirements of Sargent & Lundy (S&L) Design Specification H-2256,
" Phase II Piping Erection."
S&L is being requested to provide an engineering evaluation of the safety significance of not meeting full thread engagement, of having an undersized plug in a particular system, and of incomplete thread engagement in reducer segments of piping systems. A report will be submitted by October 31, 1982 to address the results of this evaluation.
