NRC Letter, dated June 28, 1982, Mr. T. W. Bishop to Mr.

R. S. Leddick, NRC Inspection at Washington Nuclear Project No. 3 (WNP-3).

The referenced letter reported the results of the NRC inspections, con-ducted on June 7-11, 1982, of activities authorized by NRC Construction Permit No. CPPR-154. One 10CFR50, Appendix B, Violation (subject noncom-pliance), concerning a failure to specify and perform ASME Code required surface examinations on electrical penetration field welds, was identi-fied.

Attached is a Supply System approved report detai'.ing corrective / preven-tive actions taken and the date of full compliance for the subject non-compliance. The Supply System will consider the violation to be satis-factorily resolved by the July 31, 1982, date of full compliance.

[ ,* Dated July 30, 1982 10CFR50, Appendix B, Violation (Noncompliance 50-508/82-12/02) 10CFR50, Appendix B, Criterion X and paragraph 17.1.10 of the Quality Assurance Program in Section 17 of the PSAR state in part: " ...Examina-tions, measurements, or tests of material or products processed shall be performed for each work operation where necessary to assure quality..."

Contract Specification No. 3240-224, paragraph 3.1.4.3 states in part:

...The penetration assemblies shall be installed, inspected and tested in accordance with ASME Boiler and Pressure Vessel Code,Section III, Subsection NE for Class MC Components..."

The ASME Boiler and Pressure Vessel Code 1977 Edition including addenda through Winter 1979 Section III, Subsection NE, paragraph NE5211 applies

• the requirements of NES211.1 to category B weld joints. Paragraph NE5211.1 states in part: "Where the joint detail does not permit radiographic examination, ultrasonic examination plus liquid penetrant or magnetic particle examination may be substituted for...the closing seam within an electrical penetration assembly..."

Contrary to the above requirements on June 8,1982, neither magnetic par- l ticle nor liquid penetrant examinations were specified or performed on Field Weld No. I for each of Electrical Penetration Nos. 102, 104 and 106.

Field Weld No.1 is a category B butt welded joint forming a portion of the containment pressure boundary. Weld braze data cards for these joints had been reviewed and approved by contractor personnel and the authorized nuclear inspector and had been completed and returned to qual-ity assurance records. "

This is a Severity Level IV Violation (Supplement II).

Corrective Actions Taken All Weld Braze Data Cards (WBDCs) for electrical penetrations (55 total)

-were reviewed for compliance to ASME Section III, Subsection NE. Twenty-seven (27) of the fifty-five (55) WBDCs did not indicate that sur-face examination was required. Twenty-one (21) of the twenty-seven (27) deficient WBDCs were in-process and corrected to require the surface examinations. The remaining six (6) WBDCs were documented as deficient and dispositioned to assure the required surface examinations are per-formed.

Corrective Steps Taken to Avoid Further Items of Noncompliance All relative WBDCs have been reviewed for compliance and those found deficient were corrected as required.

Personnel responsible for assigning required examinations on WBDCs have received formal documented training that included Code and Contract required inspections and the review of WBDCs to assure incorporation of these requirements. ,

Date of Full compliance The date of full compliance is July 31, 1982.
