Dear Mr. Keppier:

On January 25, 1982, Mr. T. D. Geib of Public Service Company of Indiana Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). Review of the Nuclear Steam Supply System (NSSS) support steel document packages supplied by Lakeside Bridge &

Steel Co. revealed that records were missing from several packages and the quality of the affected supports was therefore indeterminate.

PSI conducted a review of NSSS support steel document packages and retrieved many of the missing records from files at Lakeside Bridge and Steel's Shop. Review of certain retrieved radiographic records has necessitated the weld repair of some of the support steel delivered to the site. PSI's review also identified 307 weld seams for which documentation of required Nondestructive Examination (NDE) was not retrievable. Of these weld seams 244 have been examined at the site and accepted. PSI will address the remaining 63 weld seams once difficulties with accessibility are resolved.

This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before November 19, 1982.
