This report is being submitted in accordance with Technical Specification 3.3.3.3 for San Onofre Unit 2 (Facility Operating License NPF-10) which requires a Special Report within 10-days after the seismic monitoring instrumentation has-been inoperable for more than 30 days.

On December 16, 1993, surveillance testing, strong motion accelerometerswith Unit 2 in Mode 1 at 98% p(SM ower, during routine monthly were determined to be inoperable. SMA 2XT-8020B is the steam generator base support monitor and 2XT-8020E is the containment operating level monitor..

The cause of the inoperability will not be known until local inspections of the-monitors are completed. 2XT-8020B is' inaccessible during-power operations' and, as allowed by Surveillance Requirement 4.3.3.3.2, will be inspected and repaired during the next outage of sufficient duration. 2XT-8020E was scheduled to be repaired on January 18, 1994, but would have resulted in other seismic monitors also being out of service during the repairs. Due to possible aftershocks from the January 17, 1994 Northridge earthquake, these repairs were rescheduled for i

i January 26, 1994.

It should be noted that during the January 17 1994, seismic. event, the only SMA i

that actuated was the Unit I free field monitor which indicated a maximum ground l

acceleration of 0.025g in the transverse direction. This is:below the 0.05g reporting requirements of Technical Specification 4.3.3.3.2.
