Eckhardt on March 29-April 2, 1982, as documented in your letter of June 25, 1982, of activities authorized by the Nuclear Regulatory Commission i (NRC) Construction Permit Nos. ATTACHMENT A NOTICE OF VIOLATION Docket No. 50-529 Construction Permit No. CPPR-142 As a result of the Inspection conducted on March 29-April,24.1982,. and. in accordance with the NRC Enforcement Policy, 10CFR Part 2, Appendix'C, 47FR9987 (March 9, 1982), the following Violation was identified. 10CFR50, Appendix B, Criterion V, as addressed in Section 17 of the PSAR, states, in part: " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordande with these instructions, procedures or drawings....." The Bechtel Procedure WPP/QCI 251.0, " Raceway Installation", Paragraph 9.10, requires that: 9.10 In the event a design change from Project Engineering, or any authorized rework occurs after a raceway has been installed and accepted, or partially accepted, tie following criteria shall apply: j 9.10 1 Issue a new contrcl and tracking system ' raceway card with appropriate rework instructions to the field. Upon com-pletion of racework rework or design field change, the installer or responsible designee shall record the date of the re-work, or design field. change, signature and badge number. 9.10.2 The AFE shall note in the Remarks Section of the new card, the document (s) authorizing the rework. The AFE/QCE shall initial, stamp and date, respectively, completion of rework. The AFE shall indicate completion acceptance with his signature and drite (louer right-hand ',j corner) " Raceway Inspection Record". Contrary to the above, on March 31, 1982, the Inspector observed a Class 1E conduit support in the Unit 2 Charging Pump Room for Charging Pump 2MCilEP01 to be mounted to the concrete wall with four (4) llilti anchor bolts. The support was associated with Class IE " Train B" Conduit 2EZAIDBRR17 and Junction Box 2EZAIDBKKJ05. The nuts on the two lower anchor bolts for this support were observed to be only finger tight. They were backed off from the support plate approximately one to two turns. \\
Attachment A Notice of Violation Page 2 The nuts appeared to have been tightened at one time based on indications in the paint on the support and nuts. The plate and nuts appeared to have been painted after proper installation of the nuts and prior to loosening of the nuts. Also, the upper two nuts appeared to have been. tampered with as evidenced by indications in the paint. The " Raceway Inspection Record" (" Raceway Card") indicated that the support had been inspected and accepted on June 26, 1981. The Inspector found no evidence that the support was not properly installed at the time of the Inspection. It appears that the nuts on the Hilti anchor bolts were loosened at a later time. There was no documentation that these nuts had been loosened as required by Bechtel Procedure WPP/QCI 251.0, " Raceway Installation", Paragraph 9.10, as stated above. This is a Severity Level V Violation (Supplement II), applicable to Unit 2.
RESPONSE
A. Corrective Steps Taken and Results Achieved NCR EA-1964, attached, was initiated on April 2, 1982 to document the nonconforming anchor bolts and their retorque. This was con-sidered to be an isolated incident. I B. Corrective Steps That Have Been Taken to Avoid Further Noncompliance Unit Engineers, Lead Field Engineers and Superintendents were briefed j on this subject at the Unit II Staff Meeting held April 9, 1982. See attached meeting agenda. t Procedure Change Notice for WPP/QCI 251.0, attached, has been written to require the removal, reiastallation, relocation and/or modifica-tion of raceways and/or raceway supports be documented on an EE580 Card.
