Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this information will be finalized by November 5,1982, at which time a complete report will be submitted.

INTERLM REPORT - DER 82-38 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 3 I.

POTENTIAL PROBLEM Four (4) 8" butterfly valves supplied by llenry Pratt were received at the jobsite with loose bolts holding the actuator to the gear box.

During receiving inspection, one bolt from each of two valves was 4

found completely detached and lying on the shipping pallet (s).

Contact with the vendor disclosed that no_ torque values have been established for the four (4) hex head bolts securing the SMB to IIBC unit.

The valves are the containment purge system isolation valves.

II.

APPROACll TO AND STATUS OF PROPOSED RESOLUTION This deficiency report is being transmitted to the vendor for review and evaluation.

This condition will not preclude the valves from being installed.

III.

PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF Tile FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by November 5, 1982.
