FINAL REPORT - DER 82-36 DEFICIENCY EVALUATION 50.55(e) q ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 I.

DESCRIPTION OF DEFICIENCY While working in the Diesel Generator control cabinets, a Bechtel craf t i

workman accidently discovered a loose wire at Control Cabinet IJ-DGA-B02.

The control cabinets are supplied and pre-wired by Cooper Energy Services.

j Further inspection of two control cabinets, IJ-DGA-B02 and 1J-DGB-B02, revealed the following nonconformances:

(a) Fifty eight (58) 4/0 vendor cables are not crimped per specification requirements.

(One cable pulled out and' remaining cables are loose.)

Lo (b) Twenty eight (28) termination bolts at 90 VRB voltage regulators (both cabinets) are not per specification requirements (too short).

(c) Extra washers utilized at terminations (inconsistent)..

Investigation by supplier indicates that improper dies for the crimping tools were provided by AMP, the supplier of the lugs and related tooling.

The deficiently crimped cables do not meet the specification requirements for quality workmanship. This indicates a breakdown in supplier's QA/QC program since adequate inspection was not maintained.

II.

ANALYSIS OF SAFETY IMPLICATIONS The poorly crimped cables are in safety-related Class IE wiring... Failure to remain connected, or to conduct electricity, will result in failure.of the diesel generator to perform its safety function.

This deficiency is evaluated as. Reportable -under the requirements of 10CFR50.55(e).

_III.

CORRECTIVE ACTION Vendor has concur' red with remioval of lugs, and installati n of new lugs using procedure 10407-13-EM-306. NCR EG-2165 documents the rework for Unit 1.

An NCR will be initiated to document rework of Unit 2.

The vendor.

discovered loose wires in the Unit 3 panel at his shop,.which will be re-worked ~ prior to shipment.

Cooper Energy Services has been notified by letter (copy attache'd) that this deficiency is also considered Reportable under 10CFR21,.and that they should determine if ~ their control panels-for any other nuclear' projects' may contain similar Reportable deficiencies.

Dear Mr. Miklos:

Enclosed is Deficiency Evaluation Report (DER) 82-36 which describes loose wiring. Discussion with your Gary Mizer has identified the cause as being improper dies supplied with the crimping tools. We are concerned that this deficiency also indicates a breakdown in your QA/QC program since this was not discovered during manufacture. We consider this reportable to the h1C under 10CFR, Part 21, as a substantial safety hazard.
