Dear Sir:
The NRC was notified of a potentially reportable deficiency in Reference (A),
Interim Reports were transmitted by References (B), (C), (D), and (E), and a time extension was requested by Reference (F). At that time, it was estimated that a Final Report would be available by August-12, 1982.
Due to the extensive investigation and evaluation required, Revision 4 of the Interim Report is attached.
It is now expected that this information will be final-ized by November 3, 1982, at which time a complete report will be submitted.

INTERIM REPORT - DER 81-12 POTENTIAL REPORTABLE DEFICIENCY ARlZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNITS 1, 2 & 3 I.
POTENTIAL PROBLEM The pull-out fuse blocks supplied as a component of the safety related 480V Load Centers by Brown Boveri Electric, Incorporated were found to have a substantial reduccion in the mating force requirement af ter several inser-tions and removals.
This condition is considered unacceptable because there exists a question as'to whether the copper female clips provided with the fuse block are capable of performing their safety related function of maintaining contact after many removals and insertions of the fuse blocks during the life of the plant.
The equipment impacted by this condition are identified by the following equipment tag numbers:
1, 2 & 3-E-PGA-L31 1, 2 & 3-E-PGB-L32 1, 2 & 3-E-PGA-L33 1, 2 & 3-E-PGB-L34 1, 2 & 3-E-PGA-L35 1, 2 & 3-E-PGB-L36 This condition is attributed to the inadvertent annealing of the fuse clip while brazing it to its' mounting terminal during the manufacture of the fuse block assemblies by the subtier supplier Gor 1.d, Incorporated, Bellefontaine, Ohio.

APPROACll TO AND STATUS OF PROPOSED RESOLUTION Brown Boveri Electric, Incorporated agrees that the' fuse blocks are not l
acceptable as delivered.and is attempting to provide a modification. plan which will correct the condition.
T: tis supplier has also indicated that this design is unique to the PVNGS project.
Could, Incorporated, Bellefontaine, Ohio is furnishing a mechanical retainer l
which, when installed in the fuse block body, is intended to provide a-positive retaining force for the fuse clips.
The vendor has submitted a l
sample for trial; however, as indicated-by the attached Bechtel telephone notes dated July 16, 1982, the original mating problem.has not been-corrected.
In order to support theEplant startup schedule, alternate plans to replace the fuse block are being developed by Bechtel engineering.

PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF Tile FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by November 3, 1982.
