Dear Mr. Seidle :

This letter provides an interim report on the investigation and evaluation of the compromised Level II Welding Inspector examination.

This problem was reported to Region IV of the Nuclear Regulatory Com-mission as a potential 10CFR50.55(e) item on July 12, 1982.

On July 9, 1982, a copy of the Level II Welding Inspector examination and answer key was discovered in the field. An immediate investigation was initiated to determine the extent of this problem and necessary corrective action. It was determined that the test in question had been l

used since March 30, 1981, and all Level II welding inspectors who took I

this examination after that date could have had access to a copy of the test and answer key. Therefore, the certifications of these inspectors were suspended and all welds previously inspected by these inspectors were considered " suspect" until further evaluation could be performed.

A total of 67 inspectors have been identified who were certified using the examination in question. Fifty of these inspectors passed the alternate verification test and their certifications have been reinstated. Three inspectors failed the retest and have been placed in a remedial training l

program as required by Procedure AP-VI-Ol.

In addition, a sample of these I

three inspectors' work will be reviewed to assure that proper acceptance criteria were used. The remaining 14 inspectors have left the project, but an attempt will be made to locate and retest these individuals. A sample of the work of any of these 14 inspectors who do not pr.ss or take the retest will also be reviewed to assure that proper acceptance criteria I

Mr. W.C. Seidle KMLNRC 82-234 August 11, 1982 inspected welds will be evaluated after the review of work previously performed by inspectors failing to pass or take the retests.

A new series of examinations has been developed and several new examin-ation security procedures have been implemented to minimize the possibility of future compromises. In addition, Nonconformance Report ISN5349MW was initiated to document the resoltulon of this problem.

A status report on the investigation and evaluation of this matter will be sent to you on or before October 20, 1982. In the interim, please direct any questions concerning this matter to me or to Mr. Otto Maynard of my staff.
